Risk Evaluation

& Mitigation Toolkit (REM)

This toolkit takes a proactive "partnering" approach to opioid safety and assumes that patients and families want to assure that medications are used safely.

The purpose of this toolkit is to help those who work with hospice patients and their family members address the issue of substance abuse and diversion.

The Hospice Support Fund, based in Nashville, and using a mail processing center in Merrifield VA, has been soliciting by mail in several areas of Virginia requesting donations to "bring end-of-life hospice care to all Americans".  This organization is not affiliated with the Virginia Association for Hospices & Palliative Care nor is it associated with any licensed hospice in Virginia.

As far as we can determine, no hospice nor hospice patient has benefited from this organization.
